New! The Health ministry office will be open on Wednesday, February 15th from 1pm till 6pm. Going forward, the office will be open in the Parish Center one day a month (usually the 3rd Wednesday) to assist parishioners with the following:
Blood pressure checks and appropriate education
Medication box set up (Clients need to bring medication bottles and holder) If team member identifies significant medication issues, med box will not be set up and a call made to the parishioner’s primary pharmacist.)
Education on how to use a glucose meter
General information on healthy living
Assistance with reading written instructions from a health care provider
Assistance with reading a medical bill or insurance statement
Assistance with understanding lab results
Assistance with finding a primary care provider and scheduling an appointment if needed.
Assistance with how to talk to your primary care provider to get the most out of the visit
Referral to appropriate social service agencies in the community
Assistance with understanding medical power of attorney and advanced health directives.
Provide appropriate literature available in the Health Ministry Office.
